Percutaneous Endoscopic Lumbar Discectomy for Far Lateral Lumbar Disc Herniation with Degenerative Spondylolisthesis

Abstract
We report the efficacy of percutaneous endoscopic lumbar discectomy for a patient with intractable unilateral leg pain in degenerative spondylolisthesis. A 48-year-old woman with severe right buttock and right leg pain that had rapidly increased over four days was presented. The plain radiographic images showed degenerative spondylolisthesis (grade I) at L4-5 level and the magnetic resonance imaging revealed far lateral disc herniation at the same level on the right. A 46-year-old woman suffered from left buttock pain with leg pain and a limping gait abruptly occurring three days ago. The plain radiographic images showed L4-5 degenerative spondylolisthesis (grade I) and the magnetic resonance imaging showed left foraminal disc herniation at L4-5 level. In both cases, considering soft lumbar disc herniation and age with social activity of the patient, we decided to perform percutaneous endoscopic lumbar discectomy. In both cases, the herniated disc fragment was completely removed and confirmed with an magnetic resonance imaging, and previous symptoms were improved. The perioperative Visual Analogue Scale score for back and leg pain was improved and the two patients showed satisfactory outcome on the Oswestry Disability Index. In the surgical treatment of far lateral disc herniation with degenerative spondylolisthesis, percutaneous endoscopic lumbar discectomy can be a safe and effective alternative surgical option for fusion surgery.
